In the Champions League (Soccer), how do the English teams that participate get selected from their leagues?

...I though they chose the top 4 (or 5) clubs in the EPL, but I didn't know they do this while the EPL is still going on. So what's the method of the selection, and is this method the same for all the other participating countries?
